Better binding of uploads

I have suffered through multiple occasions where the model falsely attributed prompt instructions as uploaded document content and listed an incorrect filename. This indicates the uploaded document was not bound to the model context, and the model did not surface its retrieval failure to me, the user.

Playing with different prompts showed beyond a doubt that Venice.ai fails in at leas one of these ways

• The file was not attached to the model request at all

• Retrieval failed and returned an empty context

• The system substituted prompt text as “document context”

• The system forbade the model from refusing
